Registration for 2011 Riders:
Registration for the first riding session will be Feb 15th through March 15th. Please fill out forms below and send them to us. We have riding sessions; Session 1 - April 4th - May 21st Session 2 - May 30th - July 16th Session 3 - July 25th - Sep 10th Session 4 - Sep 19th - Nov 5th |
Individuals who benefit from therapeutic horseback riding have conditions such as amputation, brain injury, Down syndrome, autism, cerebral palsy, hearing, visual and speech impairment, learning disabilities, mental retardation, multiple sclerosis, muscular dystrophy and many other related challenges.
Participants should print and complete the following forms:
Liability Waiver and Release Form
Emergency Medical Treatment Form
Confidentiality Form
Photo Release Form
Participant Client Registration Form
